Jeremiah, if you don't want to approve each and every respondent you need to open the form permissions, allowing anyone with the link to open it.
On your form, top left is "Settings"
Scroll down & open "Responses".
Turn off "Restrict to users of (your domain)".

Do you just take a photo with a phone? Do they have a scanner in their app?

As you can see, it's a legitimate Google Forms link hosted on their docs.google.com domain. While I used a shortened URL initially to reduce clutter, there's no risk since you'd just be filling out a Google Form directly.
But I appreciate you emphasizing link safety!
