I can't find the topic where someone wondered if a lapsed physical card is a big deal. My DOT physical is "up" soon. I just received a letter from the Mississippi DPS explaining this. Here's some of the text from the State:
Our records indicate that your Medical Certificate will be expiring in 30 days. To maintain your Compliance with Federal Code 49 CFR part 391, you must provide your Commercial Driver License Department with a valid Medical Certificate. Failure to comply will result in a downgrade to a Regular Driver License 30 days from the date of this letter. The medical certificate has to be taken to a Commercial Driver License (CDL) Station in Person, PLEASE DO NOT MAIL OR FAX. After the Downgrade, there will be a $100.00 fee to reinstate your Commercial Driver License. If you are downgraded for more that 18 months due to medical downgrade you will have to pay the full price for your Commercial Driver License to be reissued and a $100.00 reinstatement fee.
At least in Mississippi, it doesn't look good to be lazy about this.

Any combination of vehicles with a gross combined weight rating (GCWR) of 26,001 or more pounds, providing the gross vehicle weight rating (GVWR) of the vehicle being towed is in excess of 10,000 pounds.
Any single vehicle with a GVWR of 26,001 or more pounds, or any such vehicle towing another not in excess of 10,000 pounds.
Any vehicle, regardless of size, designed to transport 16 or more persons, including the driver.
Any vehicle required by federal regulations to be placarded while transporting hazardous materials.

State and Federal DOT Officers are responsible for commercial vehicle enforcement. "The truck police" you could call them.
