Lol, not ashamed but I'm not comfortable with that and I hope they don't force me to do it.

Lol.. today at my physical, YES, I had to remove my pants. Why? I have no clue but had to put on these weird paper shorts. I'm guessing it was so the doctor could check my reflexes easier without jeans getting in the way?

I wouldn't recommend refusing to do what the physician required to do my company mandated DOT physical.

DOT:

Department Of Transportation

A department of the federal executive branch responsible for the national highways and for railroad and airline safety. It also manages Amtrak, the national railroad system, and the Coast Guard.

State and Federal DOT Officers are responsible for commercial vehicle enforcement. "The truck police" you could call them.

I understand your frustration and concern, I was not fond of the idea either. Just got mine done last week. Nice lady. There's no groping, although I do feel like she could have at least bought me a drink first lol It only takes about three seconds and it necessary. They are checking for a hernia, something that you would not be able to recognize on your own. The faster you cough, the faster it's over with and done.
