You need to "know how to drive a truck" before you do the SPE activities. So this is after completing school, which is after you get your physical and permit. The linked part had the local offices for the SPE and lists their phone numbers.

Are you doing a company sponsored training program or going to school first and then to CR England?
The company doesn't have a portion to fill out which makes the form complete. There's a very small section that you will fill out, letting them know which company you're with but it's not something a company representative has to fill in.
If you're doing everything at England, be prepared to be held back time wise while paperwork gets processed and you have to wait for your appointment with the SPE examiner. Technically, according to the FMCSA , the entire packet has to be filled out and submitted and the training certificate sent and in your hands before you do any road training.

I am about to enter my cdl school and I'm aware that I will need a SPE certificate but I'm a little confused. Do I need to have the medical evaluation completed before I start or is this something they handle when they send me for my dot physical? I am an amputee. If I need to go with this medical evaluation done before I get my permit what is the easiest way to get the evaluation done? I'm not followed my an orthopedic surgeon or anything regularly so I'm concerned it will take a long time to get an appointment with someone.
