Will I be washed up and unemployable if I answer the questions on the DOT Medical Examination Report Form as follows:

15. Fainting or passing out? Yes

Only 1 time. About 10 years ago I passed out for about 15 seconds while eating at greasy spoon restaurant but it has never happened again.

31. Have you used an illegal substance within the past two years? yes

If I take the DOT Physical say 6 months from now ( but am able to pass a hair follicle or UA test issued by my driving school which is required for enrollment ) however am honest about my past of smoking marijuana will I be sunk? From my understanding the DOT Physical does not involve a drug test yet I'm sure the results to the questionnaire would be available for the employer to read.

32. Have you ever failed a drug test or been dependent on an illegal substance? No I've only taken 1 drug test in my whole life about 16 years ago which I passed and have never been strung out on anything.

Any thoughts or comments are much appreciated.

The answer to those questions are NO...Simple Enough

HOS refers to the logbook hours of service regulations.

15 - were you seen by a medical professional - as in - taken to the ER? Or just just passed out and woke up?

31 - NO NO NO NO. If you can drop/hair test clean - NO.

32 - If you've never failed a drug screen - then NO. If you have failed a "reportable screen" - as in, in the trucking industry or other federally regulated "safety sensitive position" - then there's going to be a record of it, and you should probably answer yes.

There's a difference between honesty and professional suicide.

A lot of these question are new on the 2016 forms - and some of them are just intrusively stupid.

Any questions answered in the affirmative, are going to have to be BACKED UP with documentation from your doctor, etc. And if you don't have what they want IN HAND ALREADY - then you may get sent home.

If you have a substance abuse issue - then go deal with it BEFORE you enter the industry (not directed at you specifically Bulwinkle). You can't be snorting rails off strippers bellies the night before orientation.

Hey Rick,

No I never went to the ER on the pass out situation and I was only out for about 15 seconds. Got up dusted myself off and went home. It was a hot summer day so maybe I just got totally dehydrated or maybe something I ate or the environment at the choke and puke where it happened may have set it off. Only time in my life it has ever happened, just that one time.

I've smoked my share of weed and drank lots of beers and whiskey experimented with harder stuff which was way over 10 years ago but have never really gotten into the hard stuff on a regular basis or used any of it in over 10 years.

I'm turned my back on smoking ( none for 1 month now ) and have learned from the forum that even having a few beers after a days worth of driving is a big no no so I'm totally up for the task of not even doing that on company time while I'm out on the road.

And no I've never failed a pre employment drug test or any other drug tests in my life and will be going to a walk in clinic to get some testing done before I under go any school or DOT testing to make sure I'm clean.

I don't like to lie about anything and feel that honesty is the best policy in life is what it boils down to. People that have to tell a lie typically are fearful about something . In my case it would be the fear of not being able to drive.

For what its worth and taking it with a grain of salt from a recruiter at Celadon answering yes to questions 15 and 31 will not automatically make you unacceptable for driving school if you can show up with a CDL learners permit and DOT Medical card in hand. Is that true? I really can't say given what others have said about how their organization does business. I'm interested in what other companies have to say about the matter.

I also talked with a Doctor who is certified to perform DOT medical examines here in Missouri. Answering yes to questions 15 and 31 will not automatically disqualify you from getting a medical card. The comment was that answering yes to these questions will prompt more questions about the yes answers. I think that's a fair thing to do to make sure you're not a safety risk.

CDL:

Commercial Driver's License (CDL)

A CDL is required to drive any of the following vehicles:

  • Any combination of vehicles with a gross combined weight rating (GCWR) of 26,001 or more pounds, providing the gross vehicle weight rating (GVWR) of the vehicle being towed is in excess of 10,000 pounds.
  • Any single vehicle with a GVWR of 26,001 or more pounds, or any such vehicle towing another not in excess of 10,000 pounds.
  • Any vehicle, regardless of size, designed to transport 16 or more persons, including the driver.
  • Any vehicle required by federal regulations to be placarded while transporting hazardous materials.
