I'm not sure I'm understanding your question.
If they ask you about any accidents within the last 5 years, whether at fault or not, list them. (5 yrs and 1 month from date of application.. no) Most companies will request a copy of any accident reports. I had to provide an accident report where I was rear-ended sitting at a red light. Obviously I wasn't at fault so it was no big deal. If they ask about any citations in the last x number of years, list it. If they say last 3 years and you got a ticket 3 years and 4 months ago, don't list it.
Give them EXACTLY what they ask for, no more, no less, and you'll be fine.

I am curious to know if when I am filling out trucking company apps, if I am required to put what's on my personal insurance information or what is listed on my 10 MVR. Reason I am asking is because, is putting the insurance info, asking for more of a reason for a company to give a negative feedback and possible denial.
