You should contact both your current DMV or DPS and the state that you plan on moving to. Those government officials are paid to answer these very questions and should have access to the very information of which you are searching for on hand (up-to-date and accurate). If any forms are necessary or presrequites or criteria or limitations: They would be able to answer those questions.

It varies from state to state. Some will switch it over, only requiring a fee and others will require taking the normal state drivers written exam (not cdl). As mentioned, your best bet would be to call said state or look online to confirm their requirements. Almost all will require you to re test for your endorsements though.

In most states you can transfer your license, I have done it from PA, to Tx, to FL. All endorsements transfer except for HazMat. It requires retesting and new fingerprints and Homeland Security background. You just need proof of residency just the same as any other drivers license
