No one would rent a truck, or lend one, to someone with no license as their insurance would not cover them. That would be a dream lawsuit for an accident victim.

I ask because I had to do the downgrade once. When I was ready to start driving again, all I had to do was get a fresh DOT physical. No truck needed. Very easy.

You're going to have to go through a driving school to use a truck. They're the only ones insured to let an unlicensed driver operate their trucks. Call them up and explain your situation they'll either quote you a rate or not. Lather, rinse and repeat until you get a yes. I don't know rates in Virginia but renting a truck a driver for the day to test at the DMV runs about $800 in Washington state.

If "last June " is June 2022 you're in luck. Straight from Virginia DMV website says
What will happen if I do not self-certify or submit any required medical documents? DMV is required by law to mark any record that does not comply with this requirement as "not certified" and initiate a downgrade of your CDL privilege after 30 days of expiration. If your CDL is downgraded for more than one year and you reapply for your CDL, you will be required to complete all relevant CDL knowledge and road skills exams.
So as long as your medical card didn't expire ON/BEFORE February 6th 2022 (monday, since most offices aren't open weekends) you should only need to get an updated physical and deliver it directly to the DMV. This would not require retesting if it was only a medical card issue if it's been less than 1 year.

Please provide some additional details. How did you rent a class7/8 truck (one that requires a CDL A) to practice with when you currently have only a permit?

I stopped driving to pursue other opportunities, and foolishly let my DOT physical lapse last June. I am wondering if anyone has experience retesting without going to school. Where would I even find a truck to use for the test? I feel like not many are willing to rent a truck to someone without a license. I am in Virginia.
I know some mega companies offer refresher courses, but I believe those are for drivers with a current CDL but no recent experience.
