I’m an amputee above my right knee from 4 years ago. I’ve been doing research on getting the SPE packet filled out and I’m currently stuck on locating the Board-qualified or Board-certified physiatrist (doctor of physical medicine) or orthopedic surgeon. I’ve called numerous locations with no help at getting ahold of a physiatrist/orthopedic surgeon to complete the packet. I’m located in Texas but willing to travel if there’s a medical examiner that would be able to fill out that portion of the packet.
